Hemolysis After Pulsed-Field Ablation of Atrial Fibrillation
Xu Y, Gulburak TK, Lu YM, Zhang J, TuErhong Z, Tang B and Zhou X
Pulsed-field ablation (PFA) offers notable advantages in atrial fibrillation (AF) ablation by selectively targeting cardiomyocytes via an irreversible electroporation mechanism, thereby minimizing damage to surrounding tissues. Although clinical studies demonstrate that PFA is both safe and effective, PFA-mediated hemolysis and potential acute kidney injury (AKI) development have been recently reported. This study comprehensively reviews the literature on PFA-associated hemolysis, analyzing the underlying mechanisms, risk factors, and preventive management strategies. Additionally, the review discusses approaches to minimize the risk of PFA-induced hemolysis and AKI, while offering insights for improving PFA-associated techniques.

Clinical Importance of Tissue Proximity Indication During Pulsed Field Ablation for Atrial Fibrillation: Insights from Initial Experience

Pulsed field ablation (PFA) for paroxysmal atrial fibrillation (AF) has been gaining worldwide acceptance due to its efficacy and safety. A variable loop circular catheter (VLCC, VARIPULSE, Biosense Webster, Inc.) for PFA, recently launched in Japan, includes a Tissue Proximity Indication (TPI) feature to monitor catheter-tissue contact via impedance. However, the role of TPI during pulmonary vein (PV) isolation (PVI) is unclear.

Focal pulsed-field ablation (F-PFA) integrated in electroanatomical mapping (EAM) systems allows tailored lesion sets in patients with atrial fibrillation (AF).

The assessment of left ventricular (LV) systolic function and quantification of LV ejection fraction (EF) in patients with atrial fibrillation (AF) can be difficult. We previously demonstrated that LV volume changes over the 100 ms of systole (LVEF) can be used as a measure of LV systolic function.

Detecting atrial fibrillation (AF) after stroke is a key component of secondary prevention, but indiscriminate prolonged cardiac monitoring is costly and burdensome. Multivariable prediction models could be used to inform patient selection.

Cryoballoon Pulmonary Vein Isolation: Double Stack vs. Standard Ablation
Kibel S, Bennett MT, Bennett RG, Deyell MW, Simon F, Bennett E, Medwid L, Pearce C, Macle L and Andrade JG
The results of pulmonary vein isolation are limited by arrhythmia recurrence, which is most often due to a failure to effectuate a durable contiguous circumferential transmural lesion around the pulmonary vein (PV) ostia. We postulated that enhancing the index ablation line through use of multiple cryoballoon diameters would be superior to standard ablation with bonus freeze.

Catheter ablation is an effective treatment for atrial fibrillation (AF). Pulsed field ablation (PFA) has emerged as a novel energy modality, which relies on high-voltage electric fields to achieve cardiac tissue ablation. Data on its efficacy in the elderly are scarce.
