Myopericytoma (MPC) is a rare tumour characterized by a perivascular proliferation of pericytic cells with myoid differentiation and a typical spindle shape. Except for the rare malignant cases, MPC mostly shows a benign course. Symptoms are often non-specific, and the diagnosis could be accidental. Simple biopsies are often non-diagnostic and do not provide any information about the benign or malignant course of the disease. General agreement for its management is lacking.

Spontaneous Complete Regression of Breast Cancer: Two Case Report

Spontaneous regression (SR) is a tumor's partial or complete disappearance without any treatment. In the literature, it has been documented that SR is uncommon in breast cancer (BC) and other types of cancer. Multiple mechanisms are believed to contribute to the development of SR. However, its mechanism still needs to be clearly demonstrated. Although two SR patients were presented in our study, the evidence needed to be more sufficient to determine the mechanism. However, due to Programmed Death-Ligand 1 (PD-L1) negativity in both patients, the hypothesis in the literature that PD-L1 has strong antitumoral activity was not supported. In addition, it was determined that the patient in case 2 was the first Cerb B2 positive case reported in the literature and had the earliest SR period. Due to this, it has been disclosed that the SR mechanism of BC will be concluded within 21 days at the earliest. This situation suggests that breast surgeons, in particular, should conduct a thorough physical examination and, if necessary, re-radiological examination before surgery on patients for whom surgery is decided after diagnosis. Being careful in this regard may increase the number of SR in BC cases and allow molecular investigations on living tissue samples to reveal the underlying mechanism.
Laparoscopic Surgical Intervention for Pediatric Paraduodenal Hernia: Overview and Literature Review of a Rare Cause of Abdominal Pain and Obstruction

Paraduodenal Hernia (PDH) is an extremely rare cause of bowel obstruction. The true incidence of pediatric PDH is unknown since there are very few published cases of this phenomenon. PDH carries a high morbidity and mortality rate if unidentified. Proper diagnosis and prompt surgical intervention are extremely important. Our case is of a healthy 3-year-old male who presented with six days of generalized abdominal pain associated with constipation, anorexia, insomnia, and non-bloody, non-bilious emesis. Abdominal ultrasound (US) was negative for intussusception and failed to visualize the appendix. Abdominal x-ray (AXR) showed nonspecific distention of several loops of bowel with air-fluid levels. Computerized tomography (CT) scan was concerning for an internal hernia versus possible transverse colonic volvulus. The patient's abdominal pain and distension worsened after nasogastric (NG) tube placement. With the combination of physical and imaging findings, the patient underwent diagnostic laparoscopy. Intraoperatively, severely dilated bowel was encountered, which was decompressed via the appendix. After which, we discovered a PDH on the left side of the ligament of Treitz which was successfully reduced. The patient had no complications and continues tolerating a regular diet with return of normal bowel function. Our case describes successful laparoscopic surgical reduction of a pediatric left paraduodenal hernia.
Anesthetic Effects of Dexmedetomidine Combined with Nalbuphine in Patients Undergoing Laparoscopic Cholecystectomy and its Impact on Nutritional Status

Laparoscopic cholecystectomy (LC) is a common surgical procedure for the removal of the gallbladder. Effective anesthesia is crucial for ensuring patient comfort and safety during LC. Dexmedetomidine, a selective α2-adrenergic agonist, is widely used as an adjunct to anesthesia due to its sedative and analgesic properties. Nalbuphine, a synthetic opioid analgesic, is also employed for pain management during various surgical procedures. This study aimed to determine the anesthesia effects of dexmedetomidine combined with nalbuphine on patients undergoing LC and its impact on their nutritional status.

With 144 Retained Stones alongside Ureteropelvic Junction Obstruction: Is Stone-Free Status Possible? A Case Presentation

Ureteropelvic junction (UPJ) obstruction, in addition to causing progressive renal function loss, can lead to the formation of retained stones. Management planning for concomitant stones and UPJ obstruction is a topic of debate in the literature. We performed laparoscopic pyeloplasty and extracted all 144 stones from the renal pelvis and calyces of a 37-year-old male with UPJ stricture, using wireless flexible cystoscopic guidance. No complications occurred during the perioperative and postoperative periods. In challenging cases of UPJ obstruction with stones in multiple calyceal locations, simultaneous flexible cystoscopy offers clinicians a significant advantage.
